improve *#100# USSD reply when a subscriber has no extension

Description

It is quite possible that a subscriber in the HLR DB has no MSISDN assigned.
In that case, we shouldn't reply with "Your extension is " (stopping short) but rather something like
"You currently have no extension assigned and cannot be called, contact your operator."

I'm resolving this as a side-effect of the new *#100# handler in osmo-hlr, see osmo-hlr.git change-id I1d09fab810a6bb9ab02904de72dbc9e8a414f9f9 (not yet in gerrit review)
